Order a Birth Certificate by Mail

NOTE:

AS OF JULY 1, 2009 THE COST OF BIRTH AND DEATH CERTIFICATES WILL BE $15.00 FOR THE 1ST COPY AND $5.00 FOR EACH ADDITIONAL COPY.

SENIOR CITIZENS MAY OBTAIN A COPY OF THEIR OWN BIRTH RECORD FOR $5.00/COPY.

TO ORDER CERTIFIED BIRTH CERTIFICATES BY MAIL THE FOLLOWING IS NEEDED:
1. Write a short note giving the full name at birth (maiden name if married); Birth date; name of birth parents, including your mothers maiden name and your signature is required.

2. A photocopy of a major piece of identification, preferably a driver's license, state issued identification card or passport card.

3. A money order made out to the City of Trenton in the amount of $15.00 for the first copy and $5.00 for each additional copies per the same request for each individual record. (personal checks are not accepted)

4. A self addressed stamped envelope. ( preferably a legal size envelope)

BIRTH CERTIFICATES CAN ONLY BE ISSUED TO:

The person the certificate belongs to (if over 15 years of age)

A Birth Parent named on certificate

Legal Guardian with Permanent or Temporary Guardianship/Custody Papers

Circuit Court Order
